We welcome the opportunity to take over prenatal care in early pregnancy or provide shared care,please indicate your preference on your referral. Patients who meet the following criteria may be referred to Foothills Maternity Group.

There are two easy ways to refer to Foothills Maternity Group:

2. Fax referral letter including all patient info and relevant investigations
